Scope of Work

  • Prepare for and facilitate MORE Care meetings and calls
  • Follow up with teams on work in between calls
  • Provide technical assistance to teams (e.g. finding resources, connecting each other, and fielding questions as they arise)
  • Support state-level infrastructure efforts that impact MORE Care program

Skills/Experience needed

  • Familiarity with health care system generally, and some familiarity with oral health is preferred
  • Demonstrated success in partnership development and stakeholder engagement
  • Experience with the fundamental concepts of healthcare quality improvement
  • Comfortable working with and supporting healthcare teams in a variety of environments, specifically in primary care – and dental preferred
  • Program/initiative-based experience a plus
  • Ability to multi-task and prioritize, work autonomously, establish timelines, and meet deadlines
  • Works well in a team environment and able to generate solutions to issues as they arise
  • Bonus: Familiarity with the Institute for Healthcare Improvement (IHI)’s model for improvement, and breakthrough series collaborative

Responsibilities for MORE Care Ohio (OHO contractor)

  • Participate in an onboarding session with CareQuest Institute to share Collaborative overview, goals, and responsibilities
  • Collect and compile state-level information and data related to integration topics to share with project team and participating clinics (i.e. coding details, billing guidance/reimbursement allowances, etc.)
  • Serve as the in-state liaison between project stakeholders, physicians, dentists, rural health clinics, patients, and other social service agencies
  • Assists with the development and refinement of the program, particularly assisting in the engagement and recruitment of rural primary care and dental providers
  • Provide leadership and support for primary care practices participating in MORE Care Ohio Learning Collaborative by co-leading and co-facilitating Collaborative activities (meetings and monthly calls) as well as working one-on-one with practices as needed to support their implementation of recommended change ideas.
  • Provide coaching to participating primary care practices, including assisting them with implementing the MORE Care integration/coordination change ideas into their practice and workflow; and troubleshooting challenges that may arise during implementation
  • Update MORE Care Program tracking and reporting on a monthly basis
  • Attend and contribute to regular meetings with MORE Care planning team
  • Provide support to CareQuest Institute project staff in the collection, analysis, and dissemination of data from participating clinics
  • Review practice data on a monthly basis to ensure participants are meeting collaborative goals and inform the development of Collaborative activities
